1. Purchase qualifying Northstar 8000i 12" System during the valid mail-in rebate offer period April 1, through August 31, 2009.
2. Complete both sides of this rebate form. Customer must sign the agreement on form.
3. Mail the completed form, along with sales receipt and completed Warranty Registration form, to the address listed.
Northstar 8000i 12" Rebate valid only with eligible U.S. and Canada Northstar purchases during valid mail-in rebate offer period.
Please include with this completed form; original dated sales receipt (or copy) and completed Warranty Registration Card. Requests will be processed within 4-6 weeks.
The request must be postmarked within 30 calendar days of your purchase date (date listed on sales receipt).
